The Group Order Form is an alternative to ordering online when your order includes five or more recipients or ship-to addresses.

The form is an Excel spreadsheet with one line per gift recipient. Before using the order form, please review the instructions below.

The Excel File and Instructions can be opened directly from the links below. When you open the Excel File, first save it to your local drive. The Zip folder below contains both files. Please call or use our Contact Us Form if you have any questions about the process.
