2026 Group Order Form

The Group Order Form is an alternative to ordering online when your order includes five or more recipients or ship-to addresses.

The form is an Excel spreadsheet with one line per gift recipient. Before using the order form, please review the instructions below.

You can open the Excel file and instructions directly from the links below. When you open the Excel File, first save it to your local drive. The Zip folder below contains both files. Please call or use our Contact Us Form if you have any questions about the process.

After you complete your Group Order form, use our CMB Group Order Form Upload Form to send us the file.

After you upload or email your group order to us, we will respond within 24 hours. If you do not receive a response, please contact us by phone (888-727-7887) or use our Contact Us Form.
